Observations/Finding

At the request of the client’s representative, our AS ENGINEERING AND CONSULTING, LLC (ASEC) representative visited the job site to visually observe the contractor’s work activities from a quality standpoint to verify compliance with project drawings/specifications, applicable codes, and materials submittals and conducted QC testing per the project drawings and specifications.  The following observations were made on site this shift:

Incident Summary

A request was made to perform a proof-roll at 9:00 AM on the future deceleration lane. Upon arrival, it was observed that the future deceleration lane subgrade was not ready for proof-rolling, leading to the postponement of the scheduled activity. The foreman on-site decided to delay the proof-roll to the next day.

Sequence of Events

  1. Proof Roll Request

  2. Arrival at Site
    • Time of Arrival: 9:00 AM
    • Upon arrival, it was observed that the deceleration lane was not in a condition to perform the proof roll. Necessary preparations and compactions were incomplete.
  3. Assessment of Deceleration Lane Condition
    • Observed Condition:
      • The surface was not adequately prepared.
      • Required compaction levels were not achieved.
      • Presence of loose material and unfinished grading.
  4. Decision to Delay
    • Decision By: On-Site Foreman
    • The foreman decided to delay the proof-roll to allow for the completion of required preparations.

Analysis

  1. Preparation Issues
    • Incomplete preparation of the deceleration lane hindered the scheduled proof-roll. This includes inadequate grading, compaction, and removal of loose materials.

Immediate Actions Taken

  1. Communication
    • Confirmed the new scheduled time for the proof-roll.
  2. Preparation Plan
    • Coordinated with the foreman to ensure all necessary preparations for the deceleration lane are completed before the rescheduled proof-roll.

Conclusion

The inability to perform the proof-roll as scheduled was due to the unprepared condition of the deceleration lane subgrade. Immediate actions have been taken to address the issue, and a new proof-roll has been scheduled for the next day.
