Observations/Finding

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Avinash Bandam visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (Reference drawing E720661 S1 – R8, E720659 S1-R8.)

During the visit, the following observations were made:

1. Two thousand one hundred and six (2106) cubic yards of fill materials were placed for lift (19) in section 2 part 2 area G7 (E720659 S1-R8). The materials were placed and compacted in accordance with ITP-001, and the fill area is highlighted in brown as depicted in the attached drawing.
2. The area has been probed using hand probe and three moisture density tests and a check-plug were performed in the basement areas B7 and G7. The enclosed and attached drawing depict the test locations.
