Summary

At the request of the client’s representative, our AS ENGINEERING AND CONSULTING, LLC (ASEC) representative visited the job site to observe the contractor’s work activities from a quality standpoint to verify compliance with project drawings/specifications, applicable codes, and materials submittals and conducted QC testing per the project drawings and specifications. The following observations were made on site this shift: <strong>Summary:</strong> Upon arrival at the site, cement treated base (CTB) was being placed on lanes 5-8, with GeoHydro overseeing quality control (QC). The day's activities encountered several notable events and issues, as follows: <strong>Key Events:</strong> <ol> <li><strong>Cement Truck Issues:</strong> Initially, issues were reported with the cement trucks, as documented in the provided photo. However, these issues were later resolved, allowing work to proceed.</li> <li><strong>Pre-pour Card:</strong> No pre-pour card was completed due to an impending rain event.</li> <li><strong>Asphalt Milling:</strong> Asphalt milling was underway on the west shoulder at various depths, as illustrated in the attached photo.</li> <li><strong>Safety Meeting:</strong> A safety meeting was conducted with CW Matthews safety manager and the project manager, necessitated by a site-wide shut down.</li> <li><strong>Moisture Issue at A4:</strong> Quality Assurance (QA) identified high moisture soil around A4, prompting its removal and replacement with No. 89 stone, as evidenced in the provided photo.</li> <li><strong>Compaction Testing:</strong> <ul> <li><strong>M10 Sand (Between A2-A3):</strong> Optimum density-moisture ratio (proctor) was established at 123.9 dry density (DD) and 9.5% moisture (M). Test #1 yielded 124 DD and 5.5% M, after recommending moisture addition and recompacting, which was deemed acceptable.</li> <li><strong>Graded Aggregate Base (GAB) (Between A2-A3):</strong> (the next lift of 12 inches) The moisture-density relationship (proctor) was determined to be 138.00 pcf (dry density) and 5.7% moisture content. Initial testing resulted in 129.6 DD and 4.4% M, indicating only 94% compaction, which was below the acceptable threshold. After water was added and additional re-compaction, re-testing showed 132.35 DD and 5.6% M, meeting the 95% compaction requirement.</li> </ul> </li> <li><strong>Underdrain Elevation Issue:</strong> It was discovered that the underdrain near 972.500 was placed at an incorrect elevation. Excavation began on site to address this issue, but completion was pending as the extent of the incorrect elevation had not been determined at the time.</li> </ol> <strong>Conclusion:</strong> No further observations or testing were conducted on this day.
