<ol> <li>Introduction: A site visit was conducted by a representative from AS Engineering and Consulting (ASEC) to provide quality control inspection and testing services. The purpose of the visit was to verify compliance with project drawings/specifications, applicable codes, and materials submittals through visual observation techniques.</li> <li>Site Visit Details: <ul> <li>Attendees: ASEC Representative, Sanitary Pipe Foreman</li> </ul> </li> <li>Observations: During the site visit, the following observations were made: a. Non-Standard Means and Methods: A discussion was held with the sanitation pipe foreman regarding the non-standard means and methods being used, particularly concerning lift sizes. It was determined that no approval would be provided to the property owner due to these deviations. b. Excavation and Pipe Installation: <ul> <li>Excavation: Excavation from manhole 1.4 to 4.0 began with the installation of 8-inch green sanitary pipe on a bedding of approximately 12 inches of No. 57 stone.</li> <li>Laterals: Three laterals (078-02, 032-02, 033-02) were excavated and installed.</li> <li>Lift Sizes: <ul> <li>078-02: The first lift was approximately 2 feet in depth, with Graded Aggregate Base (GAB) used only under the future roadway, while the remaining area was filled with soil.</li> <li>Water Usage: No water was added to the initial 2-foot lift or the two subsequent 12-inch lifts of GAB during the site visit.</li> <li>Compaction: Each lift was compacted with a trench packer.</li> <li>032-02: The first lift was approximately 2-3 feet of soil.</li> </ul> </li> <li>Manhole Area: Approximately 3 feet lift of soil was placed around the manhole at 1.4 and compacted with a trench roller.</li> </ul> </li> <li>Recommendations: Based on the observations, the following recommendations are made: a. Standard Means and Methods: Ensure that all construction activities adhere to standard means and methods as specified in project drawings and specifications. b. Compliance Verification: Conduct regular inspections to verify compliance with project requirements and address any deviations promptly. c. Proper Documentation: Maintain detailed records of construction activities, including lift sizes, materials used, compaction methods, and any deviations from the standard procedures.</li> <li>Conclusion: The site visit identified deviations from standard means and methods in lift sizes and construction practices. Recommendations have been provided to address these deviations and ensure compliance with project requirements.</li> </ol> This report summarizes the observations made during the site visit and outlines recommendations to address identified issues and ensure compliance with project requirements.
