Summary

At the request of the client’s representative, our AS ENGINEERING AND CONSULTING, LLC (ASEC) representative, Amin Azimi visited the job site to observe and monitor the contractor's work activities from a quality standpoint to assure compliance with project drawings/specifications, applicable codes, and materials submittals and conducted QC testing per the project drawings and specifications. The following observations were made on site this shift: <ul> <li>Pre-pour observation performed in the Ramp 2 phase 4,Lane 2 and lane 4 portions of the project on this day. Contractor completed rebar and dowel basket placement for these lanes.</li> <li>The writer visually inspected the installation and setup of these lanes and advised the contractor representatives of any minor corrections that were needed.</li> <li>The contractor cleaned loose rock and debris and standing water from the base.</li> <li>Baskets were found to be level, straight and secured.</li> <li>Dowel bars were found to have epoxy. Embedment length appeared to be 15 inches into concrete.</li> <li>The writer along with Josi of ATLQA and ATLNext representative Hazim and CWM rep Brandon signed the pre-pour card. and also.</li> <li>Work was being performed in the Phase 4a Asphalt shoulder portion of pavement replacement project on this day.</li> <li>At this time the panels were removed and the subgrade appears to have been milled and adjusted to proposed bottom of concrete elevation.</li> </ul> <ul> <li>A fully loaded tandem axle truck was used to proof-roll the above the mentioned subgrade with multiple passes across the area. The subgrade appeared to be unyielding and little to no deflection was observed during the proof-roll in this area. Except a small area in SE corner (6x30) feet was yielding QC recommended, under cut 8 inches and replaced with structural fill or GAB.</li> <li>In addition to this, the writer along with Josi of ATLQA and ATLNext representative Brylyn performed a post demolition spall check.</li> </ul>
