Observations/Finding

AS ENGINEERING AND CONSULTING, LLC (ASEC) representative Avinash Bandam visited the site at the request of Jackson Demolition representative Josh Kelly. The undersigned representative was present to monitor, verify and document the ongoing project activities. (Reference drawing E720661 S1 – R8, E720659 S1-R8.)

During the visit, the following observations were made:

1. One thousand two hundred and forty-two (1242) cubic yards of fill materials were placed for lift (16) in section 2 part 2 area G7 (E720659 S1-R8) for the lift (16). The materials were placed and compacted in accordance with ITP-001, and the fill area is highlighted in brown as depicted in the attached drawing.
2. The basement area was probed using hand probe and performed three moisture density tests in section 1 and section 2 areas of the basement and a check plug has performed to verify the compaction, moisture content percentage with respect to the ASTM standards of the Proctor curves. The test locations are depicted in the enclosed and attached drawing.
3. The Geo-fabric and stone aggregate were placed and covered correspondingly around the HDPE pipe at the corner of the east and south end walls of the basement area G7. The placement of the Geo-fabric and stone aggregate were done in accordance with ITP-006 and ITP-003.
4. Test results are listed within the Density test report of this day.
